Advantages of Coronary Computed Tomography Angiography Over Myocardial Perfusion Imaging in Evaluating Coronary Artery Disease: A Comprehensive Meta-Analysis

Resumo

Introduction: Established as evidence, coronary computed tomography angiography (CTA) is highly accurate in assessing coronary artery disease (CAD). However, myocardial perfusion imaging (MPI) remains relevant for the functional evaluation of coronary stenoses. This meta-analysis aims to highlight the advantages of CTA compared to MPI in CAD assessment.
Method: Searches were conducted in PubMed, Embase, and Cochrane databases to identify randomized controlled trials (RCTs) comparing CTA with MPI in evaluating patients with suspected stable or acute CAD and chest pain. Odds ratio (ODD) were calculated with a 95% confidence interval (CI). R version 4.2.1 was employed for statistical analyses.
Results: We included three studies, totaling 614 patients with chest pain. Of these, 18,9% (116 patients) underwent CTA. There was no significant difference in diagnostic accuracy between CTA and MPI in patients with chest pain and intermediate risk of CAD (OR 0.65; 95% CI 0.12-3.50; p=0.619).
Conclusion: The comparative analysis between coronary computed tomography angiography (CTA) and myocardial perfusion imaging (MPI) in patients with suspected coronary artery disease (CAD) did not demonstrate statistically significant differences in diagnostic accuracy, emphasizing the importance of individualizing the method choice considering various clinical and technical factors.
